As a HealthTeam Advantage member, you can get one free in-home health assessment per year through the HouseCalls program. These personalized visits are completed by advanced practice clinicians and are intended to supplement — not replace — care from your primary care provider (PCP).
What happens during the visit?
Each visit is customized to your health needs. The clinician may do some of the following:
For urgent issues, the clinician will notify your PCP during the visit. Urgent issues include, but are not limited to, uncontrolled hypertension, new or uncontrolled diabetes, wounds, or other injuries requiring immediate care.
What happens after the visit?
After the assessment, the clinician will provide you with a checklist of suggested topics to discuss with your PCP. HouseCalls also sends a letter and Plan of Care report to you, your PCP, and your health plan. The report covers what was completed and discussed during the visit. You and your PCP are encouraged to talk about any findings at an upcoming appointment.
